1. You must be a manufacturer to file a reimbursement with Amazon using this form; otherwise, use an INVOICE.
2. Your company name and legal person name should match your Amazon store info.
3. The date should be several days before you create the shipment(s).
4. If a box contains multiple SKUs, insert an extra column and then fill in the box. (For example, if box 1 contains 3 products A, B, and C, then box 1 will have 3 rows to fill in the item name, quantity, etc.).
5. IMPORTANT - Download the PDF form and print it, then stamp it with the company seal.
